First Visit?  We surveyed new patients about what information they would like to have known prior to their first visit. We compiled these items into a special "frequently asked questions" sheet for new patients who have not made their first visit to our office. First Visit Tips! 

Paperwork needed for your new patient visit will be sent to your home prior to your appointment. It can also be downloaded at this link: New Patient Forms rev. 9-09.PDF  We must have this information before you can be seen by the doctor.

  Please complete this paperwork at home before your visit. Having these forms completed PRIOR to your first visit will save a lot of time and make your visit more pleasant.

  Please bring all of your insurance cards. If you have had any diagnostic procedures, such as MRI, X-Ray, CT, EMG, for the condition you are seeking trreatment for, please bring any reports or CD's with you. Often, the doctors will need to see these reports before they can fully recommend treatment.

  Please bring any pertinent medical records/reports from other physicians who are treating you, especially if you have been seen by another pain management physician.

  Please bring any pain medications that you are currently taking with you so we can see them. Please understand that there are some prescription medications that you can only have written by one doctor so it is important that we see exactly what medicine is being taken and precisely who is writing the prescription.

  • Severe Arthritis

  • Leg Pain
